How to fix broken windscreen demister lines?

The windscreen or windshield in the US uses warm air from the heater/AC to demist or in the US defrost. Under the dash there are hoses that connect to the heater unit and go up to dispersers at the windscreen. There's usually a separate hose for each side. Obviously a hose has come loose.
